Designated Premises Supervisor - Change of DPS
Premises Licence Holders who need to apply to change the named Designated Premises Supervisor (DPS) on a Premises Licence need to submit an application and a consent form signed by the person who is proposed as the new DPS.
The application form and the form of consent are downloadable from the link to the right.
Before submitting your application, you need to ensure that:
- Your prospective Premises Supervisor holds a personal licence
- You state on the application the date when the new DPS change is to take effect (this may be from the receipt of the application)
- You have enclosed your fee of £23 (made payable to Stratford on Avon District Council)
